One unique type of brow bone reduction is that when only the tail of the brow needs to be reduced. This may be needed in transgender brow bone reduction patients where the tail of the brow bone has been neglected in the initial surgery or in the cis-female who has a prominent outer brow bone that sticks out below the tail of the eyebrow. It can also be part of Cat Eye surgery to help get an accentuated temporal browlift effect. Bone and soft tissue facial trauma is one of the three major areas in the field of maxillofacial surgery. Having its origins in World War I and II from the last century where many facial injuries occurred, the need for specialized surgery methods for facial reconstruction began. It has now evolved into very sophisticated technologies for bone fixation and movement (plates and screws) as well as replacement of missing soft tissues. (pedicled and free flap transfers) The ‘high-rent’ district of the face allows the need for numerous medical specialities to participate including ophthalmology, neurosurgery, radiology, and dentistry in addition to the classic three surgical specialities of maxillofacial, plastic and otolaryngologic surgery. This book does an excellent job of bringing the knowledge base of all such disciplines together in a single concise volume.

Far more common are the category of Aesthetic Facial Asymmetries, which are much smaller in magnitude than the more severe Craniofacial ones, but share the underlying cause of facial development side differences. (not severe enough to be called malformations) As a result, most aesthetic facial asymmetry problems rarely affect just one facial feature or area. Often it usually affects the whole side of the face from the forehead down to the chin. It usually requires close inspection to see the full extent of the facial involvement. In most cases the asymmetry is caused by underdevelopment (hypoplasia) of one side of the face rather than overgrowth (hyperplasia) of a facial side. Patients may only see the facial feature that is most asymmetric but almost always other feature asymmetries exist. One of the best ways to see the extent of facial asymmetries are 3D CT scans. With the bone as the foundation of most of the external facial features, such scans usually provide meaningful insight into what is seen on the outside.

Even if a man has not had breast enlargement during adolescence, breast tissue can still develop due to pharmacologic and supplement influences. Common examples include marijuana use, drugs such as propecia, steroids and chemotherapy agents, numerous body building supplements and weight gain Most gynecomastias present at this age with a protruding nipple due to an underlying glandular breast mass or larger and softer overall chest enlargement.

Wound Care
All incisions used for migraine surgery, either in the upper eyelid or in the hairline, are closed with dissolvable sutures. No dressings or wound care are needed. Migraine surgery patients can shower and wash their hair the very next day. There will be some mild discomfort at the scalp incision sites, but the swelling and bruising is very minimal. When upper eyelid incisions are used, one can expect some moderate upper eyelid swelling for a week after surgery. There are no physical restrictions or limitations after surgery. Some numbness will be present around the incision sites for a few months. Most patients experience an immediate improvement in their migraine headaches although the full benefit of the migraine surgery may take weeks to be fully realized.
Ear Surgery
The ears are examined and the form and shape of the ear analyzed. The effectiveness of the procedure can be determined by folding the rim of the ear back and seeing if this gives the desired shape in protruding ear patients. Whether tissue grafts may be needed to reconstruct the ear is determined at this time. Photographs are taken prior to surgery for documentation. Patients should wash their hair the night before surgery.
Orthopedic Surgery
One of the most common questions about shoulder reduction surgery is in the amount of bone that can be removed and what effect it creates. The visible change in shoulder width occurs as a result of both horizontal bone length reduction as well as some forward rotation of the shoulders. The rounding effect of the shoulders comes from the anterior rotation which appears to be relatively minor as it relates to perceived width reduction.The typical amount of clavicle bone removed is between 2 to 3 cms based on patient body size and natural shoulder width.The question of how much bone can be safely removed is based on its effect on bone healing and postoperative shoulder function. Extrapolating from the orthopedic surgery experience in clavicle fractures, bone shortening in the 2.5cm+ range is not associated with abnormal scapulohumeral kinematics. This indicates that no negative shoulder functions should occur in elective clavicle length reductions in the range of 2 to 3cms. What would happen beyond 3cms to 3.5cms of clavicle length loss in a bilateral procedure has unknown effects for shoulder function.

Also remember that operative includes other events than actual surgery. The measurement of operative time starts the second you walk into the operating room and doesn’t stop ticking until you are wheeled out on a stretcher. Neck Lift
One other approach to the sagging neck is that of the direct neck lift. Unlike facelifts in which the incisions and the direction of skin lifting is based around the ears, the direct neck lift removes loose neck skin by excising it down the center of the neck. This is a simpler approach to a neck lift and is a very powerful reshaping method of the neck, but it does so with the trade-off of a midline neck scar. This can be a preferred procedure for older men (over the age of 65) who prefer the least recovery and have large hanging neck wattles. Male beard skin heals remarkably well and I have not found the neck scar to be a visible concern after it heals.
Blepharoplasty
The concerns about an ‘overdone’ look and eyes that may look very unnatural can occur from combined upper and lower blepharoplasties if they are done aggressively. Too much eyelid skin removal can also cause dry eye symptoms from delayed or incomplete closure of the eyelids and slowing of the blink reflex. Nose Surgery
Most chin implants are placed under IV sedation or general anesthesia as an outpatient. They are commonly placed through a very small skin incision under the chin where the bone is easily reached. Placing the implant through a small incision inside the mouth behind the lower lip can also be done. Chin bone advancement is always through an incision in the mouth where the bone is cut, brought forward, and fixed into its new position with a small plate and screws. Often times, chin procedures are part of other facial procedures including nose surgery and facelifts.
Body Lift
One common misconception about buttock lifts, and admittedly the use of term ‘lift’ in its name adds to this perception, is that it is done from the top part of the buttocks and the lower part is thus lifted. While such a procedure exists in bariatric plastic surgery as part of a circumferential body lift, this is the exception as to how the procedure is performed. It is performed at the bottom of the buttocks with lifting of the infragluteal fold level but not overall buttock mound. This is why it may be better to call the procedure an excisional tuck. But at the least it should be call a lower buttock lift.

Otoplasty
Its surgical correction is an ear reshaping procedure called otoplasty which has often been called a ‘pinning back’ of the ears. In this ear procedure, the antihelical fold of the ear is recreated by sutures, the size of the concha reduced or weakened, or a combination of both techniques is used from an incision on the back of the ear. While it is commonly performed in children (can be done as early as two years old) and teenagers, it can be done just as successfully in older adults. There is no upper age limit at which an otoplasty can be successfully performed. The changes that result from otoplasty can be some of the most dramatic of all ear reshaping surgeries.

Lip Augmentation
Injectable fillers are the most common method of lip enhancement. For the lips, the injectable filler should be composed of hyaluronic-acid, a naturally resorbing sugar-like molecule (e.g., Juvederm, Restylane), that has very few problems and always goes away. While losing the injected filler volume is not desired by patients, the lips are very sensitive and potentially reactive to particulated longer-lasting fillers and these should be avoided for lip augmentation. While injectable filler lip enhancement is quick to-do and produces an instant result with virtually no recovery, treatments must be repeated and the cost over time can be significant. As a result many patients will eventually develop ‘filler fatigue) and seek a more permanent solution to their aesthetic lip concerns.

Laser Resurfacing
PRP (platelet-rich plasma) is a natural concentrate of one’s own blood that contains platelets and other growth factors. By drawing a small amount of blood, it is centrifuged and a concentration or plug of platelets is obtained. This can be injected into wounds, mixed with other wound healing agents, or sprayed onto the surface of the wound. Because it is natural and contains potent healing agents, it has become popular for many wound and surgical procedures. It is often used today as an adjunct to such aesthetic procedures as facelifts, facial laser resurfacing, fat grafting and hair revitalization.

Dermabrasion
2) and erbium:yag ( Er:YAG) laser vaporization has become popular in recent years. Through selective ablation of water-containing tissue, both laser systems offer predictable reproducible vaporization of tissue, resulting in better control compared to dermabrasion. Their effects are directed to ‘resurfacing’ the edges of the scars in an effort to either decrease the topographic differences between the level of the scar and the surrounding normal skin. (either lower the surrounding rim of normal skin or lower the raised scar portion) Whether laser resurfacing will be beneficial depends upon how discrepant the differences are between the scar and the surrounding skin. As a general rule, differences of up to 1mm can be treated with visible signs of improvement. CO2 laser resurfacing is more powerful but results in greater tissue removal and longer recovery periods. Er:Yag laser resurfacing has become more popular today due less recovery but it also creates less of an improvement per single treatment and usually requires a series of laser resurfacing treatments to see its best scar benefits.
Facial Implants
The technological developments in medical biomaterials and an improved understanding of the influence of hard and soft tissue on the face has made the use of facial implants a routine procedure predictable results. While once reserved mainly for facial bone deformities, they are now an integral part of contemporary management of facial aging as well as such conditions as facial asymmetry and for facial reshaping. Facial implants come in two basic materials, silicone (which is by far the most commonly used) and medpor (porous polyethylene). Each material has its own advantages and disadvantages, neither facial implant material is perfect. Regardless of the implant’s composition, they are all placed in a subperiosteal position right next to the bone. Most of the time, the implant is secured to the bone by small screws so there is no chance of shifting or implant migration.
Bariatric Surgery
Lack of fullness of the buttocks, which often appears as a ‘flat butt’ without a rounder shape, is the most common reason today for unhappiness with the shape of the buttock area. Some patients who undergo buttock augmentation have a congenitally small buttock, have lost a significant amount of weight after bariatric surgery, or simply desire a much rounder and fuller buttock than what they have. This concern can be treated with either fat injections or a buttock (gluteal) implant, depending upon the size and area of the buttock deficiency.

The main surgical options to be considered include elevation of the eyebrows through small incisions in the scalp through a specially designed scope (endoscopic forehead lift), elevation of the eyebrows and forehead through an incision along the frontal hairline (pretrichial forehead lift), or elevation of the brows through the eyelid approach (transbleparoplasty or transpalpebral browlift), or combinations of any of these procedures. Q: Dr. Eppley, I have one vertical scar on forehead and one atrophic scar (dent; ) on top of nose which has broken my profile line. Forehead scar from a cyst and nose scar from a biopsy caused by sunburn incident..which caused skin cancer. Can you fix the scars and how can my profile be fixed. I feel really bad that I no longer have a smooth profile line. Also, I avoid photos because my dent catches the light which emphasizes it. Lasers? Surgery? Cost? I am not interested in fillers but would prefer a permanent fix.

Atrophic scars are skin depressions most commonly caused by the destruction of collagen, a primary skin component that gives skin its elasticity and firmness, during inflammatory skin reactions like chickenpox, trauma, or surgery. These pitted or indented scars make the skin appear much thinner and often red due to its thinness and small vascular telangectasias in them. Trying to apply makeup to conceal the lesions can actually worsen the skin’s appearance as makeup enhances the textural variations.
Chemical Peels
To deal with smaller amounts of loose lower eyelid skin, a more limited form of a transcutaneous lower eyelid lift is done. Under the lash line, skin is pinched up using a special instrument and then removed without undermining the entire lower eyelid skin. This can be combined with chemical peels or laser resurfacing at the same time as the blood supply to the eyelid skin has not been disrupted. (unlike a full transcutaneous lower eyelid lift) This pinch lower eyelid lift is used in younger patients who have early onset eyelid aging or as a touchup or maintenance procedure for those who have already had a prior eyelid lift procedure.

While men and women face similar aging issues of the face, facial rejuvenation approaches are quite different and reflect certain anatomical needs as well as gender differences. Men undergo less facelift procedures and they must be done very carefully to hide the scars and not make visible changes to the temporal and occipital hairlines and the facial beard skin. Blepharoplasties (eyelid tucks) need to be done more conservatively and brow lifts very judiciously to avoid a ‘done’ look and over-feminizing the male face. Men do not do as much skin care and Botox and injectable fillers treatments as women and seek more definitive surgical options rather than non-surgical options that require maintenance. But when it comes to wrinkles and skin care, men seek for a natural reduction not a complete elimination of them.
