An oral exam is performed on both new and existing patients to determine their dental and health status. New patients receive a more comprehensive exam that includes x-rays and checks for gum and bone disease, systemic disorders and oral cancer. A more routine exam is done for existing patients to see if there have been any changes in health since their last visit. In order to look for gum disease, the gum tissue is measured with a small ruler to measure the pocket depth between the tooth and connective tissue. A measurement of over 4mm could indicate disease or infection. Gum disease can develop easier in deeper pockets due to the extent that plaque collects in these deep pockets. If the oral exam reveals the need for tooth scaling or root planing, these services are performed in lieu of or in addition to the routine cleaning.

WHAT ARE THE RISKS, BENEFITS AND ALTERNATIVES? The benefits of teledentistry include having access to a dentist and additional dental information without having to travel to a dental office or clinic. Some of the procedures that you may receive include X-Rays, cleaning, fluoride treatments, sealants or temporary fillings. A potential risk of teledentistry is that a face-to-face consultation with a dentist may still be necessary after the teledentistry appointment. This could be because of your specific medical or dental condition or for other reasons. Recommendations will be made to you about your future dental care after the teledentistry consultation. These could include recommendations about whether or not to see a dentist in a dental office or dental clinic. A visit to a dental office may be needed in the future even if it is not recommended now. The recommendations may change if more information about your dental needs becomes known. The alternative to teledentistry consultation is a face-to-face visit with a dentist.
Dental Sealants

Fillings
The most common is a composite filling, this is a natural tooth colored filling and bonds to the tooth for extra strength. There are also gold and silver fillings. The final option is a porcelain filling, also called an inlay, which is the most durable of fillings and is also the color of your natural teeth.

Veneers
A veneer procedure is a popular cosmetic procedure. Porcelain is known as one of the best materials for esthetic dental procedures, because it mimics tooth enamel very closely. Veneers are extremely thin (usually less than 0.5 mm thickness), giving them enamel-like translucency, creating a natural appearance. They are about as thick as a contact lens. There is very little tooth preparation before the veneer application.
Crowns
To place a crown, your dentist must reduce 1-2 mm of the tooth to make room for it. Your dentist will then use a piece of thread or cord or use a laser to push the gum down around the tooth, to take an impression of the tooth. The impressions are sent to the lab where the crown is made. During that time, you will have a temporary crown. These crowns are usually made of plastic and are made in your dentist's office on the day of your visit. They are not meant to last. If a temporary crown is left in the mouth, the cement eventually washes out and the tooth can decay. At a second visit, your dentist will remove the temporary crown and test the permanent one. Sometimes crowns need additional polishing, glaze or some other adjustment before they are placed. Once the crown is ready, it's cemented to your tooth.

Porcelain veneers are attached to your teeth using a dental bonding glue or cement. Although veneers are very thin, once bonded to your teeth they become strong. The process is fast and conservative. First, your teeth are prepared and impressions are sent to a dental laboratory along with the design for your new smile. Root Canal Treatment
The development of cavities is usually in those hard to see and reach places in your mouth. These are the areas that usually floss can only reach. Cavities are formed when the bacteria in your mouth attaches to food particles and forms plaque. The acid produced by plaque begins to eat away at the tooth if it is left long enough. First the enamel is attacked then the dentin is next, which is where the tooth's nerve fibers are located. Once the decay makes its way to the dentin a filling is need to stop the cavities progress. If it is left untouched it will eventually reach the nerve and a root canal is then needed to repair the tooth.
Apicoectomy
Sometimes the infection persists even after root canal therapy. In these cases, an apicoectomy, or root end resection can be performed to remove diseased tissue.

Flap Surgery
When deep pockets between teeth and gums (6 millimeters or deeper) are present, it is difficult for a dentist to thoroughly remove the plaque and tartar. Flap surgery is a procedure where the gum flap is lifted away from the tooth. Diseased tissue and sometimes bone is removed. The rough surfaces of the tooth are then smoothed by root planing. The area is medicated and the gum flap is replaced and sutured allowing the bone and gum tissue to heal. One of the goals of flap surgery is to reduce the depth of the periodontal pockets to make them easier to keep clean.

Tooth loss occurs for many reasons; dental decay, root canal problems, fractures, periodontal disease and trauma. Additionally at times teeth never develop. As a result of tooth loss the bone melts away. If there is inadequate bone to support surrounding teeth and future possible dental implant(s), bone grafting materials are used to rebuild and regenerate the bone providing sufficient bone in quantity and quality for implant placements and supporting tooth structure.

Dr. Mireles graduated from the University of Southern California with an honors degree in Psychobiology in 1983. Dr. Mireles then went on to receive his dental degree from the University of California at San Francisco in 1987 where he also received special training in Oral Medicine. At the end of 1993 he finished a two year post-Graduate Training Program in Orthodontics. In 2003 he finished a one year course on advanced surgically assisted orthodontics.
